We want an in-progress response to show only in its own chat. New Chat during a stream should show the empty welcome screen, switching chats should show only that chat, and the original chat should update when its answer finishes even if you're not looking at it.

If you click New Chat or open another chat while a response is still coming in, the "Thinking..." status or the partial answer shows up on the screen you just opened. The app doesn't tie the in-progress response to the chat it belongs to, so it shows wherever you are. When the answer finishes, it also refreshes the chat you're looking at instead of the one that was answering. We want an in-progress response to show only in its own chat.
